First Dental Visit
Your first dental visit will consist of a comprehensive exam, cleaning and xrays. In order for us to provide you with the comprehensive dental care you deserve, check- up xrays (4 Bitewings and 2 Periapical xrays) are taken once per year and a full mouth set taken once every five years.
Exam and Cleaning
Our office focuses on prevention and maintenance of your oral health. To ensure that diagnosis of treatment is identified at an early and more conservative stage, the doctor will examine your teeth and review your xrays at each cleaning.
Treatment Presentation
After your comprehensive exam is completed, if treatment is needed, your personalized treatment plan will be reviewed with you. The treatment plan consists of an overview of the recommended treatment. Patients with insurance will receive an estimate reflecting the insurance benefits applied towards the recommended treatment and the patient's copay.
Co-Payment
Payment is expected at the time services are provided. Financial arrangements can be made with our office in advance.
For extensive treatment requiring more time, we ask that a non-refundable deposit in the amount of 10% of the total treatment be provided to reserve the appointment.
Cancellations/ Rescheduling
Since appointed times are reserved exclusively for each patient, we ask that you please notify our office 48 hours in advance of your scheduled appointment time if you are unable to keep your appointment. Another patient in need of dental care could be scheduled if we have sufficient time to notify them.
Insurance
We currently accept Assurant, Cigna, Delta, First Dental Health, Guardian, Metlife, United Concordia, Humana, Principal, and Connection Dental. Our office does not participate in Health Management Organizations (HMOs).
Please keep us informed of any insurance changes such as policy name, insurance company address, or a change of employment status.
We file dental insurance as a courtesy to our patients. We are not responsible for how your insurance company handles its claims or for what benefits they pay on a claim. We will assist you in estimating your portion of the cost of treatment, but we cannot guarantee what your insurance will or will not do with each claim. We make every attempt to file claims accurately and to resolve errors if they occur. We are more than happy to serve as a secondary resource for any questions you may have regarding your dental insurance.
Emergency Services
Dr. Bliesath is dedicated to providing emergency care to her patients and understands that dental emergencies can occur after office hours and on weekends. Should you have a dental emergency after office hours, please do not hesitate to contact the doctor via our answering service which will provide you with the doctor's direct contact information.
